Somerset County United Way is committed to improving the quality of life for all residents of Somerset County. Building on our expertise and experience in addressing community issues, Somerset County United Way is expanding its work to dig more deeply into pressing issues, identifying root causes, engaging new partners, and developing lasting solutions that will make life better here in Somerset County.

Currently, Somerset County United Way assists the community by providing information on a Prescription Drug Savings Program. Somerset County United Way will be identifying additional ways to expand the work we do toward this area of need in the near future.

Discount Prescription Programs

 

Families and individuals who do not have insurance coverage for prescription drugs can now receive discounted prescriptions, thanks to partnerships with leading organizations. The cards are free and provide discounts of 20-40% off leading prescription drug costs, including many generics. Savings vary by pharmacy.

 

FamilyWize

The FamilyWize discount prescription card is a free resource from the FamilyWize community service partnership for those who have no prescription drug coverage. There are no eligibility requirements other than not having any insurance coverage for prescription drugs.

 

Simply present your card and prescription to a participating pharmacy to obtain the discounted price. You always receive the lower of the discounted price or pharmacy's retail price.

 

Use your card as often as you need, for any prescription, for everyone in your family. There is no income limit, age requirement, eligibility or registration required.

 

The goal of this community partnership is to reduce the cost of medicine for children, families and individuals with no prescription drug coverage. FamilyWize cards are being distributed free of charge nationwide by participating United Ways, community and faith-based organizations, county agencies, doctors, businesses, some pharmacies, and individuals like you. The discounts and savings are provided voluntarily by the more than 53,000 participating pharmacies that have agreed to accept this card. Together Rx Access

The Together Rx Access® Card was created as a public service by a group of some of the world's largest pharmaceutical companies, in order to provide savings on prescriptions to eligible residents of the US and Puerto Rico who have no prescription drug coverage.

 

Individuals and families who meet all four of the following requirements are eligible for the Together Rx Access Card:

·          not eligible for medicare

·          have no public or private prescription drug coverage

·          household income equal to or less than:

o         $30,000 for a single person

o         $40,000 for a family of two

o         $50,000 for a family of three

o         $60,000 for a family of four

o         $70,000 for a family of five

o         legal resident of the US or Puerto Rico

 

Once you receive your Together Rx Access® Card, activate it and within hours, present it when paying for included prescriptions at participating pharmacies, and you’ll save instantly, right at the pharmacy counter.

There are no maximum usage limits, no hidden fees and no monthly charges.

Somerset County United Way also invests a portion of funds entrusted to it in programs that promote healthy lifestyles for individuals and families.

 

All funded programs must establish desired outcomes that are achievable, measurable and generate overall impact area outcomes.  Listed below are examples of the types of services funded in healthy lifestyles and sample outcomes from some of those programs. 

 

Living a Healthy Life

 

Vision: Individuals and families will lead physically and emotionally healthy lives that enable them to achieve a living income.

 

Programs that provide needed physical and mental health care services, improve health care access, link people with needed services, prevent illness and promote wellness.  Services include:

 

·          Education, nutrition, legal and financial advice and services

·          Adult day care

·          Preventative health education, screenings and awareness

·          Provision of information, access and referral to quality health, medical and wellness programs

·          Substance abuse prevention, treatment, rehabilitation programs, along with other support needed by the individual/family

·          Physical and mental health support

·          Improve access to health care for uninsured or underinsured persons

 

                                Impact Area Outcomes

 

o         Individuals make choices that ensure good health so that they can achieve educational/employment success and financial stability.

 

Sample program outcomes:

§          Couples and families will improve or resolve their relationship issues leading to healthier and more productive lives for the family members.

 

§          Clients will maintain recovery from substance use disorders through new skill development and behavior changes, making them better prepared to obtain/maintain employment/education

 

o         Individuals have access to appropriate healthcare and treatment in order to resolve health issues that are preventing educational success, employment and financial stability.

 

Sample program outcomes:

§          Patients will have access to health care services, regardless of ability to pay.

 

§          Clients obtain and preserve health care access and treatment that make it possible to achieve educational success, earn a sustainable wage, and be financially secure.
